- The 7 Guidelines for Documentation
o State the specific disability
- The diagnosis should be made by a professional
o Be current
- In most cases, evaluation and diagnostic testing should have taken place within 5 years of the request
- For psychiatric disabilities, the diagnostic should have taken place within 12 months of the request
- For visual disabilities the diagnostic should have taken place within 2 years of the request
o Provide relevant education, developmental, and medical history
o Describe the comprehensive testing and techniques, including the test results with the sub test scores
o Explain how the disability effects the student’s daily functioning
o Describe the specific accommodations requested
o Provide the professional credentials of the evaluator (e.g. certification, license…)
There are two ways a student can be determined eligible for accommodations on tests
School verification

- A request is submitted by the student’s school through SSD online, and submitted to the College Board
- School must submit:
• SSD Coordinator’s form
• School Testing Calendar
• Parent consent form
• Student Eligibility form
- Process from start to finish can take approximately 3 months
- In special cases, students or parents can submit a request for accommodations without the involvement of the school by submitting a Student Eligibility Form
Documentation Review

- If students are ineligible through the School Verification Process they may still be eligible through Documentation Review. In order to find out, they must submit documentation with their eligibility form.
- Students must complete sections I and III of the request for accommodation form at SSD online
- They should then print out the fax cover
- Attach appropriate documentation to the fax and then send it to the College Board at the fax number listed
- Once a student has been approved for accommodations they will receive an SSD number listed on their SSD online dashboard. Students should use this number when registering for College Board tests.
- If a student has been approved after already having registered for the SAT, verify that the registration has been updated by calling SSD customer service at: (609) 771-7137

*Student’s will not know in advance if their request will need to be reviewed, so it is in their best interest to submit their request by the earliest deadline
